Endor Holocaust -- according to one of the Star Wars Tales comics from Dark Horse, this in-story urban legend was debunked, and the true story is that most of the DSII's mass was vaporized in the explosion, and the rest of the debris was intercepted by Alliance starships.

I don't know if the Dark Horse comics are considered "canon," however.

Post-google edit: "Apocalypse Endor was a comic appearing in Star Wars Tales 14 and collected in Star Wars Tales Volume 4." Also, the comics are "C-canon."
